Referral Process
Firstly, your doctor will order the home health care based on the medical necessity of your needs. From there, our administrative team will verify your insurance eligibility. You will then be assigned a registered nurse who will come to your home and perform an evaluation to determine your health care needs based on the Medicare medical necessity criteria. After that, our home health professionals will work with you, your doctor, and any available caregiver to create a plan of care that will provide services to meet your specific needs.
